Think you're too old to have your teeth straightened? While we automatically pair “teenager” with “braces,” at least one in five orthodontic patients are adults. And there's many more that could benefit, as many as three-quarters of adults with a correctable bite problem.
But although orthodontics can be performed at any age, it's not a minor undertaking. It will require time, patience and expense. So, before you decide to undergo orthodontics, here are 3 simple questions to ask first.
Why? Like children and teenagers, adults can benefit cosmetically from correcting a poor bite. But there's another great reason besides a more attractive smile: misaligned teeth are more difficult to care for than normal teeth. Orthodontic treatment is an investment and potential cost-saver in your future dental health.
Why not? Even senior adults can successfully undergo treatment. But braces might be ill-advised if you have either poor oral or general health. Periodontal (gum) disease, for example, can cause bone loss, which makes it difficult to safely and successfully move teeth (and the effort could worsen current disease activity in the gums). Medical conditions like bleeding disorders, leukemia or uncontrollable diabetes could interfere as well. You'll need both a dental and medical examination beforehand.
How? We can use braces — or we might be able to use a newer, more popular option with adults called clear aligners. These are a series of computer-designed clear, plastic trays you wear in sequence until you finish the series. Each tray is slightly smaller than the previous tray, moving the teeth in much the same manner as braces. But unlike braces, you can remove aligners for cleaning or a rare special occasion — and they're much less noticeable than metal braces. Although in some cases braces may still be the best option, it's also possible clear aligners could be the option you've been looking for.

’Tis the season for holiday joy with sweet treats at every turn. Don’t let it be the season for dental woes as well. You've heard that sugar causes cavities. That’s because bacteria in your mouth feed on sugar and release acid as a by-product. The acid eats away at tooth enamel, resulting in tooth decay if not checked. To protect your smile during the December onslaught of cookies, candies and other goodies, follow these tips:
Seek balance. Foods that stick to your teeth like candy canes, chewy candies or potato chips provide more opportunity for cavities to develop. To help keep your smile sparkling for the New Year, mix it up with healthy options. Chances are you will come across tooth-healthy offerings like raw vegetables, a cheese plate or mixed nuts. Vegetables scrub your teeth while you chew and stimulate the production of saliva, which helps neutralize acid and rebuild tooth enamel. Cheese also neutralizes acid in the mouth and has minerals that strengthen teeth, while nuts stimulate saliva production and provide vitamins and minerals that keep teeth strong and healthy.
Consider your timing. There’s a higher risk of developing tooth decay when sweets are consumed as standalone snacks, so when you do eat sugary treats, try to have them at mealtime. Repeated snacking between meals exposes teeth to food particles throughout the day, and the acids produced can continue to act on your teeth for 20 minutes after a treat is consumed. During meals, however, other foods present help balance out the sugar and stimulate saliva production, which helps neutralize acid and wash away food particles, sugar and acid from your teeth.
Watch what you drink. Sipping sweet drinks over time can have ill effects on your teeth because of prolonged contact with sugar. If you consume sugary beverages, try to do so in moderation and preferably along with a meal. Sipping your drink through a straw can help keep the beverage away from direct contact with your teeth. Consider opting for water—there are plenty of other opportunities for extra sugar and calories! Besides, water washes away food bits and dilutes acidity. After eating the sweet stuff, it’s a good idea to drink water or at the very least swish a little water around in your mouth.
Keep up good oral hygiene. With all the holiday busyness—shopping, gatherings with friends and family, school functions—you may find yourself exhausted at the end of the day. Still, this is an especially important time to keep up your oral hygiene routine. Brushing your teeth with fluoride toothpaste morning and night and flossing every day are key to keeping your teeth for the long haul.

Sometimes it seems that appearances count for everything—especially in Hollywood. But just recently, Lonnie Chaviz, the 10-year-old actor who plays young Randall on the hit TV show This Is Us, delivered a powerful message about accepting differences in body image. And the whole issue was triggered by negative social media comments about his smile.
Lonnie has a noticeable diastema—that is, a gap between his two front teeth; this condition is commonly seen in children, but is less common in adults. There are plenty of celebrities who aren’t bothered by the excess space between their front teeth, such as Michael Strahan, Lauren Hutton and Vanessa Paradis. However, there are also many people who choose to close the gap for cosmetic or functional reasons.
Unfortunately, Lonnie had been on the receiving end of unkind comments about the appearance of his smile. But instead of getting angry, the young actor posted a thoughtful reply via Instagram video, in which he said: “I could get my gap fixed. Braces can fix this, but like, can you fix your heart, though?”
Lonnie is raising an important point: Making fun of how someone looks shows a terrible lack of compassion. Besides, each person’s smile is uniquely their own, and getting it “fixed” is a matter of personal choice. It’s true that in most circumstances, if the gap between the front teeth doesn’t shrink as you age and you decide you want to close it, orthodontic appliances like braces can do the job. Sometimes, a too-big gap can make it more difficult to eat and to pronounce some words. In other situations, it’s simply a question of aesthetics—some like it; others would prefer to live without it.
There’s a flip side to this issue as well. When teeth need to be replaced, many people opt to have their smile restored just the way it was, rather than in some “ideal” manner. That could mean that their dentures are specially fabricated with a space between the front teeth, or the crowns of their dental implants are spaced farther apart than they normally would be. For these folks, the “imperfection” is so much a part of their unique identity that changing it just seems wrong.
So if you’re satisfied with the way your smile looks, all you need to do is keep up with daily brushing and flossing, and come in for regular checkups and cleanings to keep it healthy and bright. You’ve invested a lot of time and money in orthodontic treatment to improve your smile. If you’re not careful, though, your teeth could actually move back to their old positions. The reason why is related to the same natural tooth-moving mechanism we use to straighten teeth in the first place.
Teeth are held in place by an elastic, fibrous tissue called the periodontal ligament lying between the teeth and the jawbone and attaching to both with tiny collagen fibers. The periodontal ligament allows for incremental tooth movement in response to pressure generated around the teeth, as when we chew (or while wearing braces).
Unfortunately, this process can work in reverse. Out of a kind of “muscle memory,” the teeth can revert to the older positions once there’s no more pressure from the removed braces. You could eventually be right back where you started.
To avoid this, we have to employ measures to hold or “retain” the teeth in their new positions for some time after the braces come off. That’s why we have you wear a dental appliance called a retainer, which maintains tooth position to prevent a relapse. Depending on what’s best for your situation, this could be a removable retainer or one that’s fixed to the teeth.
Patients typically wear a retainer around the clock in the immediate period after braces, and then eventually taper off to just nighttime wear. Younger patients must wear one for several months until the new teeth positions become more secure and the chances of a rebound diminish. For older patients who’ve matured past the jaw development stage, though, wearing a retainer may be a permanent necessity to protect their smile.
Retainer wear can be an annoyance, but it’s an absolute necessity. Think of it as insurance on your investment in a new, more attractive smile.

We’ve developed a number of effective treatments for periodontal (gum) disease. Depending on how far and deep a patient’s infection has advanced, treatment can be quite invasive and even require surgery. The more invasive, the longer and more uncomfortable the healing process can be.
But using a medical laser could make that less so. Although its use for gum disease treatment is still in its infancy, the latest observations from the field seem to show patients undergoing laser treatment may have less tissue trauma and bleeding, less discomfort after the procedure and quicker healing times.
Gum disease is a bacterial infection mostly caused by dental plaque, a thin film of food particles that build up on teeth in the absence of effective oral hygiene. The infection can advance deep below the gum line, weakening gum attachment to teeth and destroying supporting bone. Ultimately the affected teeth can be lost.
Traditionally, the only way to stop the disease is to manually remove plaque buildup on teeth and gum surfaces, which is continuing to sustain the infection, with special hand instruments called scalers or ultrasonic equipment. Because it’s important to remove as much plaque and diseased tissue as possible, we may need to perform a surgical procedure called flap surgery to move some of the gum tissues out of the way to get to these deeper areas. As with any surgery, this can create tissue trauma that may cause discomfort during the healing process.
Our new alternative is to use an Nd:YAG medical laser in a procedure known as Laser Assisted New Attachment Procedure or LANAP. With light energy delivered through a small fiber no more than the width of three human hairs, the laser can pinpoint diseased tissue and destroy bacteria through intense heat. Because of the laser beam’s tiny width and pulsing action, healthy tissue is at less risk for trauma than with the traditional treatment.
Coupled with other techniques, LANAP procedures could remove as much infected tissue and plaque as traditional methods, but with less healthy tissue trauma. In the future, then, patients with advanced gum disease undergoing laser treatment could have less bleeding and discomfort and faster healing times.
